Cancellation & Payment Policy

In order to maintain the integrity of my practice, I ask that all cancellations be made with a minimum of 24 hours notice. Failure to provide 24-hour notice will result in a charge of $40.00, failure to show or call will result in a charge of $90.00.

We require payment after services are rendered. We accept insurance as long as you have acupuncture on your plan. Please note, that you are responsible for any remainder of your bill, in the event your insurance company does not pay. 

INFORMED CONSENT FOR CHINESE MEDICAL TREATMENT 

Treatments will be held by Megan Nielsen L.Ac., and/or any other licensed acupuncturists serving as back-up for Integrated acupuncture.

In the practice of acupuncture, there are some risks to treatment, including but not limited to a possible bruising, possible hematoma, residual soreness, nausea or infection. In the practice of cupping; possible bruising, in the practice of moxibustion; possible burns and in the practice of massage and acupressure; possible soreness. 
Please be sure to ask questions or raise any concerns before the treatment starts.

Unusual risks of acupuncture include spontaneous miscarriage, nerve damage and organ puncture, including lung puncture (pneumothorax). Infection is another possible risk, although we sterile disposable needles and maintain a clean and safe environment. Burns and/or scarring are a potential risk of moxibustion.


While this document describes the major risks of treatment other side effects and risks may occur. The herbs and nutritional supplements (which are from plant, mineral, and animal sources) that have been recommended are traditionally considered safe in the practice of Chinese Medicine, although some may be toxic in large doses. Some herbs may be inappropriate during pregnancy. Some possible side effects of taking herbs are nausea, gas, stomach ache, vomiting, headache, diarrhoea, rashes, hives, and tingling of the tongue.

Please immediately notify the acupuncturist of any unanticipated or unpleasant effects associated with the consumption of the herbs.

Notify the acupuncturist who is caring for you if you are or becoming pregnant. The acupuncturist will exercise judgment during the course of treatment which the acupuncturist thinks at the time, based upon the facts then known, is in the client’s best interest. Results are not guaranteed. Our office medical and administrative staff may review any medical records and lab reports, all your records will be kept confidential and will not be released without any written consent.
